Beginning in 2009, conference certificates have been created and distributed via EAPA's conference registration system.  Within 4-6 weeks after each conference, each attendee has been sent a link to their certificate of attendance.  The system creates a single certificate detailing conference sessions attended. Continuing education (CE) contact hour language is added to those who have paid for CE contact hours.

Certificates may be revised or re-issued at no cost through December 31st of the conference year.  After that, a $15 administrative fee is required.  The instructions below may assist you in retrieving your 2012 Conference certificate.  For prior years, please use this form  to purchase a replacement certificate.

2012 Annual World EAP Conference Certificates are Now Available!

Please check your e-mail for a message from j.price@eapassn.org with the subject "PDH/CEU Certificate Information - EAPA's 2012 Annual World EAP Conference" sent November 8, 2012.  This message includes a link to access the conference attendee service center and a unique username and randomly generated password required to access your certificate. 

The body of the e-mail message includes the following information:

  Username: your_email@yourdomain 

Password: xxxxxx

Please note: 

• PDH/CE contact hours are awarded for every session for which we have two scans and your scans indicate you attended a minimum of 85% of the session time.
• Any credit appeals or requests for corrections must be made in writing via e-mail prior to December 15, 2012. Please address all requests to Jan Price at j.price@eapassn.org
• If you are missing a session, please include the missing session code and time you entered and exited the session (example 1027-1a-020 entered 10:45 exited 12:15) in the body of your e-mail message.  A list of session codes, titles and times can be found in your Conference Brochure. 
• If you purchased continuing education credit, your certificate will contain both PDH and CE contact hour information. Look under the list of sessions for CE authorization.

Please download the certificate file today and retain it for your records.

After December 31, 2012, corrections, re-production or re-issuing this certificate will incur a $15.00 administrative fee.

Pre-Conference Training Certificates were distributed on-site, and are not included in the Certificate of Attendence for the main conference.  If you need an electronic replacement copy, please include the title of the full-day training(s) you attended along with your request.
